GLP-1 and Pregnancy: What You Need to Know Before Family Planning

GLP-1 receptor agonists are contraindicated during pregnancy and breastfeeding. If you are pregnant, planning to become pregnant, or breastfeeding, you must stop GLP-1 medication and discuss alternatives with your prescriber. This is not a discretionary recommendation — it is the official guidance from all major regulatory bodies worldwide.

Why GLP-1 Is Contraindicated in Pregnancy

Animal studies showed adverse effects on foetal development at clinical doses. While robust human safety data in pregnancy is not yet available (pregnant women were excluded from clinical trials), the existing preclinical signal is enough to make the risk unacceptable. The potential harm to the developing baby outweighs any benefit during pregnancy.

How Long Before Trying to Conceive Should You Stop?

  • Semaglutide (Ozempic, Wegovy): Stop at least 2 months (8 weeks) before attempting conception — it has a half-life of approximately 1 week and takes several weeks to clear
  • Tirzepatide (Mounjaro, Zepbound): Similar — stop at least 2 months before
  • Liraglutide (Saxenda): Shorter half-life (~13 hours), but discontinue before attempting conception and discuss timing with your doctor
  • Always confirm the specific washout period with your prescriber — guidelines may be updated as more data emerges

GLP-1 and Fertility: The Positive Side

For women with obesity-related infertility or polycystic ovary syndrome (PCOS), the weight loss achieved with GLP-1 medications can significantly improve fertility. Weight loss of 5–10% can restore ovulation, regularise menstrual cycles, and reduce insulin resistance — all of which improve natural conception rates and IVF outcomes. GLP-1 may thus be used deliberately as a fertility-preparation tool before pregnancy.

PCOS and GLP-1: A Special Note

PCOS is the most common cause of female infertility and is closely linked to insulin resistance. GLP-1 medications address both excess weight and insulin sensitivity — two of the core drivers of PCOS. Some women with PCOS experience improved cycle regularity within months of starting GLP-1. This also means that fertility can return unexpectedly, so reliable contraception is essential while on treatment.

Contraception While on GLP-1

If you are of reproductive age and not actively trying to conceive, use effective contraception while on GLP-1 medications. Oral contraceptive pills are generally not affected by GLP-1, though the slower gastric emptying may theoretically affect absorption timing — a barrier method or long-acting contraception (IUD, implant) removes this concern entirely.

What Happens to Weight After Stopping for Pregnancy?

After stopping GLP-1, appetite typically returns and some weight regain is expected. Managing nutrition carefully during this period — particularly through whole foods, adequate protein, and avoiding ultra-processed foods — can minimise regain. Most prescribers will revisit GLP-1 options after birth (and after any breastfeeding period ends).

If family planning is on your horizon within the next 1–2 years, discuss a timeline with your prescriber when you start GLP-1 treatment. Planning your treatment arc around your reproductive goals makes the transition much smoother.
